This is official form 2801-11 from the Immigration and Naturalization Department. These cards appear to have been used for the business of the Naturalization Court.
There are several versions of this card, all having slight variations in the layout format. Both of these cards exhibit a rearrangement of the corner card elements. The first is identified as form 2851-11. A second card no longer makes reference to Department of Commerce, instead labelling it Department of Labor. This one is form 259-14.
When I found that I had different versions of the same card, it appeared to be possible to decode part of the form numbers. I had always thought that the two-digit portion was a revision number of some sort, but after looking at these closely-related cards, this seems less likely.
